What to Bring to the Appointment:
 | Patient insurance card |
 | Any physician referral forms if required by insurance |
 | A list of current prescription or over-the-counter medications, including dose and frequency |
 | Any records or x-rays related to your problem from your primary care or referring physician |
 | Any forms that were mailed to you by our office |
New Patients
Appointments should be made via telephone or in person. Please have your insurance information available to give to the receptionist if you call. If you are currently covered under a managed care plan, we can schedule your appointment more efficiently if you provide us a physician referral number. (You will need to contact your primary care physician and ask for a referral in
advance.)
